I was at a meeting recently and they started talking about the wearing -off effect…some people piped up that it was all in the minds of some people and that when you do a proper controlled study as opposed to listening to Mr/Mrs Big Mouth then people can’t find any thing different but others say there are too many people you say the same thing that they feel like they know when they need the next dose…One MSers I know said when they were taking natalizumab that they knew when their MS-coke needed a top-up. What is the wearing off effect and what is the biology?. I thought it must be a cytokine thing creating a fatigue, but hey what do I knoe What do you think

Background: Wearing-off symptoms (WoS) are common in multiple sclerosis (MS) patients treated with biologicals.

Objectives: To investigate the occurrence of WoS in patients with MS who switch from natalizumab to ocrelizumab treatment.

Methods: We evaluated WoS in patients who completed a WoS questionnaire during both natalizumab and ocrelizumab treatment.

Results: Among the included patients (N = 33), those with WoS during natalizumab treatment had higher odds of experiencing WoS during subsequent ocrelizumab treatment, compared to those without WoS during natalizumab treatment.

Conclusions: These findings could be valuable for patient counselling, though validation in a larger cohort is desired.
